CLL HEALTH is pleased to introduce its Integrated Model of Care, marking an important milestone in advancing its integrated healthcare delivery network. The Model was formally ratified by the Council for Integrated Healthcare Advancement (CIHA) on 6 October 2026.

The CLL HEALTH Integrated Model of Care establishes a unified framework for how care is delivered, coordinated and continuously advanced across the CLL HEALTH network. Anchored in a Client-Centred Culture of Caring (4Cs), underpinned by a culture of RESPECT and delivered through Population Health-Oriented Integrated Care, the Model aims to advance the health, well-being and longevity of individuals and communities.

The Model is built around four interconnected core components: Care Delivery, Care Environment, Systems and Supplies, and Digital Integration.

Together, these components form the foundation of a connected, adaptive and future-ready healthcare system, supporting CLL HEALTH’s strategic aim of delivering better outcomes, enhanced experience and sustainable value-based care.

By integrating care, education, research and innovation across clinical and non-clinical services, the Model reinforces CLL HEALTH’s commitment to delivering coordinated, high-quality care, and creating lasting value for patients, families and communities.
